Paracentesisis/ tapping of anterior chamber


Paracentesis of anterior chamber is a procedure to open the
anterior chamber and drain its contents for various diagnostic and therapeutic
procedures.





Indications


Diagnostic sampling : endophth , ocular lymphoma


To reduce IOP in hyphema , CRAO postop pr spikes and chemical injury



To reconstruct flat anterior chamber








TECQ:


the patient to lie supine under the microscope, insertion of
a lid speculum and preincision of the cornea with a 15° micro sharp blade, and
the aqueous was aspirated using a 27 gauge needle on a tuberculin syringe.


To recontrucr flat ac balanced salt solution is injected

an aqueous pipette is used to aspirate the AC contents





CLOSURE NOT NECESSARY .. IF WOUND DOESN’T CLOSE SPONTANEO
CLOSE WITH 10 – 0 nylon suture


CAUTION


Pressure over eyeball to be maintained during needle
insertion and pipette insertion inorder to avoid entry of air bubble into AC


Needle inserted should not touch the lens surface !!!


POST OP


Apply antibiotic eye drops 4 times a day for 5 days

Complication :


1 cataract formation secondary to injury to lens


2 bleeding due to injury to iris tissue


3 corneal endothelial damage


4 descemet membrane detachment


5 persistent aqueous humor leak thru paracentesisi
